After installing the program, simply open your PDF in Foxit. Try Foxit Reader 4.3, which allows you to add text to any PDF. For example, the Phone textbox should always start with "+45" and after that the user can add the phone number. What you need is a program that's a little more accommodating, PDF-wise.
